Adoption Application

Thank you for your interest in Golden Rescue. The adoption application requests information about you and your family including contact information, information about your home, how you will care for the dog and your experience with dogs. This information is kept strictly confidential and is used for the purpose of finding the right home for a particular golden. BEFORE completing this application, please check the box to the left of the following statements to indicate that you have read and agree with each one:

	Adoption homes are selected on an individual basis to best meet the needs of the rescued Golden Retriever. A potential adopter's circumstances & preferences are taken into consideration in this selection process.
	Some of our rescue dogs have been abandoned or unwanted, and come to us with little or no history. In the best interest of all concerned, we feel it is prudent not to place them in a home with small children under 8 years of age.
	A securely fenced yard or invisible fencing is required. Tying a dog out to a post or clothesline and/or promising to always accompany the dog are not acceptable substitutes.
	Our adoption fee for a Golden Retriever 7 years of age and under is $400.00. An age appropriate fee is most appreciated for our senior Goldens. All the necessary veterinary care is provided before adoption, including all vaccinations, micro-chipping and spaying or neutering.
	The dog must not be left alone longer than 8 hours at any given time.
	I acknowledge that the information contained in this form is true and correct to the best of my knowledge. I understand that any misrepresentation of fact may result in the removal of the adopted dog from my home by the Canadian Golden Retriever Adoption Service Inc.
